$350 Initial Visit + $115/month
(6-month commitment, then month-to-month)

A flexible option if you prefer to schedule visits as needed.  Services are billed individually, and follow-up is arranged based on your needs over time.

Pricing & Membership FAQs

  • The initial visit fee is due at the time of booking.
    For membership plans, your monthly fee begins one month after your initial appointment and renews automatically on the same day each month.

  • Yes. Membership requires a 6-month commitment.

    • The initial visit fee is paid at booking

    • This is followed by 5 monthly payments

    • After the initial 6 months, membership continues month-to-month

  • After the initial 6-month commitment, you may cancel your membership with 30 days notice by emailing welcome@iconahealth.com or calling 205-791-5210.

  • No. You may choose to pay per visit. 

    However, most patients choose membership because hormone therapy often requires ongoing adjustments and support to achieve the best results.

  • Hormone therapy is a process—not a single visit.

    Most importantly, membership allows your care to be proactive rather than reactive—helping you feel better sooner and maintain results over time.

  • Appointments canceled with less than 24 hours notice will incur a $50 rebooking fee.

  • ICONA Health is a direct-pay practice and does not accept insurance. This allows us to provide longer visits, more personalized care, and direct access to your provider without the limitations of insurance-based models.

  • Yes. Many patients use HSA or FSA funds for visits, lab work, and membership fees. We recommend checking with your specific plan for eligibility.

  • No. Medications (including hormone therapy and GLP-1s) are not included and are filled through your pharmacy or our partner pharmacy. This allows for flexibility in treatment options and pharmacy selection.

  • At this time, we are not able to see Medicare patients due to current regulations. We anticipate this changing in the near future and will update patients as soon as that becomes available.

  • ICONA Health does not complete prior authorizations. While some medications prescribed may be covered by insurance, coverage varies widely by plan and medication. Our direct-pay model allows us to focus on timely, personalized care without delays related to insurance approval processes.

  • No. ICONA is a specialty clinic focused on hormone health, sexual wellness, and medically guided weight loss. We recommend maintaining a relationship with a primary care provider for general medical needs.
